I am Caring for Someone in a Residential Aged Care Facility
EPC provides specialist palliative care to people with a life limiting or terminal illness in a RACF in the last six months of life
EPC services can only be provided to people who live in one of the following areas:
- Boroondara
- Manningham
- Maroondah
- Whitehorse
- Monash
- Knox
- Yarra Ranges
If the aged care facility where the person you care for lives is not in our area:
You can call the Palliative Care Advice Service on 1800 360 000 for free, confidential advice about a life-limiting illness, palliative or end-of-life care; 7 am to 10 pm, seven days a week.
If the person you are caring for meets these criteria and you would like to refer them to the EPC program, please click below to complete a Referral Form. Please note that you can only refer someone if they have consented for you to do so.

EPC Aged Care Palliative Care Consult Service
Nursing Consult Service
Specialist palliative care nurses provide support and advice to Residential Aged Care Facility nursing staff. Consult services are provided Monday to Friday with visits arranged if clinically indicated.
A 24 hour on-call telephone support service is available to support Residential Aged Care Facility nursing staff when residents are on our program.
Nursing advice is aimed at relieving symptoms and promoting comfort to enhance quality of life. EPC Nurses work with other team members to respond to the needs of clients and their caregivers through assessment, planning, education and clinical care.
Medical Care
Primary medical care is provided by the client’s GP. EPC has specialist palliative care clinicians to provide support and advice to GPs and the EPC Care Team.
Family and Carer Psychosocial Support
Support is available if recommended by the EPC Registered Nurse.
